Print the fronts, flip the sheet, print the backs — and the backs land a millimeter or two off from where they should. Every print-and-play designer hits this. Here's why it happens and the two fixes that actually work.
It's not your file. Duplex misalignment almost always comes from the physical printer, not the PDF. When you manually flip a sheet of paper and feed it back in — or even when a printer's built-in duplexer does it automatically — the paper doesn't re-enter at exactly the same position it left. Roller grip, paper curl, and feed-tray tolerances all introduce a small, consistent offset: usually somewhere in the range of half a millimeter to a few millimeters, and usually the same offset every time on a given printer.
That consistency is the important part. It means the offset isn't random noise you have to fight sheet by sheet — it's a fixed quantity you can measure once and compensate for forever, on that printer.
The most bulletproof fix skips duplex printing entirely. In a gutter-fold layout, each card's front and back are placed on the same side of the paper, mirrored across a central fold line. You print one pass (single-sided), fold the sheet down the middle, glue the two halves together, then cut out the cards.
Because the front and back came off the printer in the same pass, there's no second feed to introduce drift. The registration is guaranteed by the geometry of the fold, not by printer precision. This is the method print-and-play veterans reach for when alignment matters more than paper efficiency — it uses roughly double the paper of duplex printing, since each card's front and back both occupy space on the same sheet instead of a whole second sheet.
If you want true duplex printing (fronts on one sheet, backs on the reverse — half the paper of gutter-fold), calibration is the fix. The idea: print a one-page test sheet double-sided, with small crosshair marks on the front and several candidate crosshair positions on the back. Hold the printed sheet up to a light. Exactly one of those back crosshairs will line up with the front one. That match tells you your printer's exact offset in both directions (left/right and up/down).
Once you know the offset, every future duplex export can shift the back pages by that exact amount before printing — canceling out the drift before it ever hits paper. This only works if the same code path draws both the calibration sheet and your actual card backs; if a tool uses one placement routine for the test sheet and a different one for real exports, the calibration doesn't actually transfer.
